どう (, how) + (いた)しまして (itashimashite, doing, humble (れん)(よう)(けい) (ren'yōkei, stem or continuative form) of the verb (いた) (itasu)). Literally, doing/having done what, ostensibly elliptical of a phrase such as, “doing what did I earn this appreciation?”, implying that the speaker did not do anything so worthy of the other person's thanks.
