- ^ Guaraní play their home matches at Estadio Defensores del Chaco, Asunción (originally scheduled at Estadio Arsenio Erico, Asunción) instead of their regular stadium Estadio Rogelio Livieres, Asunción which is being rebuilt.[11][12]
- ^ Boston River played their home matches at Estadio Centenario, Montevideo instead of their regular stadium Estadio Campeones Olímpicos, Florida which did not meet CONMEBOL requirements.
- ^ The Independiente v Universidad de Chile second leg match was abandoned in the 47th minute of play due to lack of security conditions.[13] On 4 September 2025, CONMEBOL's Disciplinary Commission disqualified Independiente from the competition.[14]
